Malaria is one of the diseases that are responsible for the highest mortality and morbidity rates in Africa especially among children (World Bank, 2009). In fact, it is one of the greatest public health concerns in Africa and most of the health programs are aimed at prevention and treatment of the disease (World Bank, 2009). This disease is common among Africans compared to other ethnic groups due to economic, cultural, economic and social factors. In a bid to fight this scourge, Africans have been using several methods to control the spread of malaria and the methods have included both the modern and traditional approaches. Malaria is an infectious disease which is usually caused by a parasite that is known as plasmodium. These parasites are protozoan in nature and they are of several species which include Plasmodium falciparum, Plasmodium malariae, Plasmodium ovale, and Plasmodium vivax (Sherman, 1998). Among these species, the most serious and fatal species to human beings is P. falciparum. The plasmodium is transmitted to humans by the female anopheles mosquitoes and its lifecycle involves two hosts which are the human host and a mosquito vector (Sherman, 1998). In the lifecycle of the plasmodium it forms sporozoites which are found in the gut of the female mosquito (Russel & Wolfe, 2008). The female mosquito transmits the sporozoites to human beings through a bite. These sporozoites move into the human liver where they enter the liver cells and mature into a schizont which contains numerous merozoites (Russel & Wolfe, 2008). These merozoites are released into bloodstream where they invade the red cells thereby forming schizonts with numerous merozoites. These are released from the red blood cells into bloodstream where they invade more red cells. As the red cells are bursting to release the merozoites, toxic compounds are released which causes the fever and the clinical symptoms that are associated with malaria (Russel & Wolfe, 2008). In the bloodstream, some of the merozoites differentiate into the male and female gametes which are taken up by the mosquito from the infected person and these two fertilize each other in the gut of the mosquito and they develop into sporozoites (Russel & Wolfe, 2008). These are transmitted to another human being through a bite by the mosquito. Though the principal mode of malaria transmission is by mosquito bites, there are other methods that can transmit the malarial parasite. One of these is blood transfusion following existence of dormant plasmodium parasites in the donor’s blood. This can make the transfused person to suffer from a febrile illness and for these reasons in areas where malaria is endemic a full course of chloroquine is administered to potential recipients of blood (Kakkilaya, 2006). Another mode of transmission is transmission from mother to child among pregnant women. These parasites pass to the child through the placenta especially if the mother has no immunity (Kakkilaya, 2006). The last mode of transmission is through needle stick injury which can be either accidental as happens among healthcare providers or intentional as happens among drug addicts who share needles (Kakkilaya, 2006). There are several factors that make Africans vulnerable to malaria and one of these are the environmental factors which contribute to spread of the disease. To begin with, malaria is a climate related disease where it is mostly found in the tropic and subtropic regions. The climate in Africa particularly the annual mean temperature is mostly within the tolerance limit of the plasmodium species and this makes the parasite to thrive thus rendering the populations living in this area vulnerable (Leary, 2008). Another environmental factor is weather disturbances in Africa which influences the breeding sites of the vectors thereby increasing the transmission potential of malaria (Leary, 2008). These weather disturbances occur in the form of prolonged droughts and heavy rains. There are several social and cultural factors that make Africans vulnerable to malaria disease. One of these is increased human population in Africa and this has led to swamp reclamation and deforestation in a bid to find more land for settlement. The effect of these activities has been the creation of puddles which provides good breeding sites for the mosquitoes which in turn transmit malaria (Leary, 2008). When the vegetation is removed, what happens is that the temperatures increase and this aids in malaria transmission. Another factor is self medication where many people in Africa buy drugs over the counter and treat themselves at home (Leary, 2008). This has led to development of drug resistant strains of plasmodium necessitating continuous replacement of anti-malarial drugs. This has made the populations in Africa susceptible to the disease since instead of dealing with the problem the populations create more problems by creating drug resistant strains of plasmodium. In addition, many people treat themselves with anti-malarial drugs that have already been ruled as ineffective thereby putting themselves at risk of developing serious and complicated malaria (Leary, 2008). Another factor is lack of knowledge on the disease among both the communities and the public health officials. A study done in 2004 showed that in East Africa people are required by the Public Health Act to clear the bushes around their houses as a way of preventing the spread of yellow fever yet studies have shown that clearing of bushes creates favorable breeding conditions for malaria (Leary, 2008). This increases the vulnerability of Africans to malaria disease since effective measures are not taken to prevent spread of malaria. Economic factors also increase the vulnerability of Africans to malaria. Poverty levels in Africa are high which means that there are inadequate economic resources to invest in healthcare thereby making people vulnerable to malaria epidemics. Most of the populations here live below a dollar a day and also cases of food shortages are very common which makes obtaining food rather than malaria prevention a priority (Leary, 2008). These economic hardships also make it difficult for populations to seek good healthcare services. Most just go to the private clinics or to the local dispensaries most of which have no equipment for diagnosis thus resulting in inappropriate prescriptions (Leary, 2008). In addition, some of the medical staff found in these healthcare facilities are not qualified. The reason as to why most of the people prefer the local dispensaries is cost constraints where they cannot afford motorized transport and thus prefer to use cheap means of transport such as bicycles (Leary, 2008). Myths and cultural beliefs also make Africans vulnerable to malaria. This is particularly with regard to the cause and prevention of malaria. Some people believe that malaria is caused by witchcraft or supernatural forces and a good example is Uganda where the populations here associate convulsions which are a form of malarial complication with supernatural forces (Leary, 2008). This really makes the populations vulnerable to malaria since they cannot take measures to prevent malaria. In addition, due to such beliefs, people who have malaria are not treated with conventional medicine which is more effective but are treated using traditional medicine which is not that effective. This makes the populations susceptible to malaria epidemics since the problem is not being addressed thereby ensuring the spread of the disease and increasing mortality rates. Other people associate malaria with certain foods and a good example is populations in Tanzania who believe that malaria is due to consumption of maize meal (Leary, 2008). This belief results from the fact that maize meals in Tanzania are commonly consumed in times of food shortages which occur following too much or too little rainfall (Leary, 2008). These climatic conditions are also associated with increased cases of malaria. However, the populations associate malaria with the food they commonly eat at that time and therefore they do not eat maize meals as a way of fighting malaria (Leary, 2008). This only serves to make them more vulnerable to malaria. There are several methods that are used to control the spread of malaria. Some of these methods are aimed at lowering exposure to infectious mosquito bites and these include indoor spraying using insecticides, use of bed nets that are insecticide treated, and use of repellants (Falola & Heaton, 2007). Insecticides are used to reduce the number of adult mosquitoes while materials such as repellants repel mosquitoes away from an individual thereby curbing the spread of the disease. Another method involves treatment of the disease using drugs. Though drug resistance has been a great problem in fighting malaria, drugs have proven to be effective in suppressing the parasite within the host thus preventing further spread of the disease (Falola & Heaton, 2007). Other treatment methods involve interfering with the breeding sites of mosquitoes and these include draining of stagnant water and spraying of breeding sites with insecticides. Apart from these methods of preventing the spread of malaria, the African population has its alternative methods of dealing with malaria. One of these involves use of traditional medicine. Traditional healers in Africa diagnose and treat malaria using indigenous roots, herbs, and leaves which are usually prepared and taken orally (Maslove et al. , 2009). This acts as a barrier to the fight against malaria since their diagnosis is based merely on symptoms and not diagnostic evidence which may lead to improper treatment thus further spread of malaria due to delayed treatment (Maslove et al. , 2009). Traditionally, Africans have relied on good sanitation practices such as proper disposal of garbage and draining of swamps as a way of preventing malaria and up to date their method remains effective and aids the fight against malaria (Falola & Heaton, 2007). However, this approach ignores mosquitoes as the vectors involved in the spread of malaria thereby hampering the fight against malaria (Falola & Heaton, 2007). Though Africans have taken steps to address malaria, a lot still remains to be done. There is a great need for education on the causes of malaria as by understanding the causes of malaria preventive and treatment strategies can be effectively implemented. In the United States of America, the health care spending has been increasing continuously (Hawe, 2008). There have been a lot of funds kept by the government of United States of America in its budget in relation to the health care services.In the year 1990, the expenditure made by the government of United States of America in relation to the health care spending was $714 million. Since then, the amount of expenditure on health care has increase to $2. 3 trillion. This is around 3 times of what it was in the year 1990. The government of United States of America is spending around 16% of its total gross domestic production on providing the health care facilities to the people. It is estimated that a round $7600 are spent by the government ofUnited States of America for each and every resident in the country. In this way, it can be said that the expenditure in relation to health care in United States of America is quite good and the country is taking adequate steps for making sure that adequate health care facilities are provided to the people in a proper and appropriate manner. The health care spending of the United States of America is the highest all around the world. There is no problem with the extent of money spend by the country towards the health care.Other countries around the world are also spending some money towards the health care but the expenditure made by the government of United States of America is the highest. Currently the expenditure incurred in United States of America towards is adequate and appropriate. It has to increase only with the increase in population. According to the per capita expenditure, the current expenditure in the country towards the healt hcare is appropriate. When the country makes the budget in relation to the health care expenditure, then it has to take care that adequate expenditure is incurred on each and every activity related to health care.The government of United States of America should make sure that it spends appropriate expenditure on each and every area of the health care. The expenditure should be more on the critical areas and it should be reduced on the areas which are not very difficult to handle for the government. Atmosphere and Setting

The novel The Reluctant Fundamentalist by Mohsin Hamid, follows the protagonist Changez’ journey through American business fundamentalism. Hamid utilises a plethora of surroundings that convey specific feelings, throughout the novel to heighten the emotions felt by Changez. This is first evident through New York City’s drastic change from optimistic to hostile, following the 9/11 attacks, which correlates with Changez understanding himself. Further as Changez is through his travels exposed to difference countries such as Greece and the Philippines, he becomes aware of the disparity between Americans and himself.This eventually leading him back to Lahore where he is disgusted to realise that he has become a â€Å"foreigner†. Shortly after . time in Valpariso ultimately leaves him disenchanted with American. Hamid creates such environments and atmospheres to amplify Changez conflicting beliefs about his place in America and Pakistan. The changes in atmosphere and s etting of New York City, mirror Changez changing attitude towards America and his identity in the milieu of New York City.Before the 9/11 attacks New York is depicted as a society that is diverse, modern and full of opportunity. The city itself exudes a feeling of encouragement and optimistic, welcoming individuals of all cultures. Hamid portrays, the city like this to consequently allow Changez, the protagonist to feel as though he belongs. Changez feel at home, in New York because of the taxi drivers speaking Urdu, the Punjab deli and his skin colour falling â€Å"in the middle of the colour spectrum†. He remarks that he â€Å"never [felt] like an American†, but â€Å"immediately [felt like] a New Yorker.To Changez, New York offers him an opportunity to achieve the â€Å"American dream†, it made â€Å"everything possible†. It is because of these specific feelings that Changez experiences towards New York City, that he is infatuated with America. Howeve r after 9/11 New York becomes of city of patriotism and paranoia, much more seclusive and guarded. At this point in the novel, Changez has begun to see the inhumanity of the pragmatic and materialistic America that he has accepted. Hamid, changes the atmosphere of New York, to further amplify Changez' dislike for America.Changez feels as through the city is saying â€Å"We are America-not New York†, making him uncertain of his place, as he never really felt like an â€Å"American†. In addition, New York's tense atmosphere provokes rumours of racism: Pakistani cab drivers being beaten†¦.. the FBI raiding mosques†¦ Muslim men.. disappearing. As a result this atmosphere makes Changez feel uncomfortable and unsure about where he stands in America. Changez time in Greece and the Manila provides the perfect enviorment for him to become increasingly aware of the disparity between Americans and himself.Hamid's use of Greece is significant, due to its laid back, relax ing atmosphere that juxtaposes against New York. This provides the ideal environment to stimulate Changez' American companions, to exhibit their preconceived American superiority. Changez becomes aware of the â€Å"ease at which they parted with money†, and their â€Å"self righteousness†, as though they were part of the â€Å"ruling class†. He becomes increasing attentive of this self satisfaction that American's carry with them. Further, Hamid uses Manila because of its similarity to Pakistan and its idolisation of Americans.Thus, Changez finds that, â€Å"it was one thing to accept that New York was more wealthy than Lahore, but quite another to swallow the fact that Manila was as well†. This setting evokes pride and jealousy in Changez. Hence, he attempts to â€Å"act and speak†¦. more like an American†¦ [he] wanted [his] share of that respect†. Despite his belief that his â€Å"Pakistaniness was invisible†, the glare that he r eceives from a driver of a jeppney, with its â€Å"undisguised hostility†, shakes Changez. He consequently realises, that this driver has seen through him and this once again makes him feel as though he is betraying Pakistan.The atmosphere in Lahore and Valparaiso plays an important role in concreting Changez disenchantment with America. When Changez first returns back to Lahore, he notices â€Å"how shabby [his] house appeared†, he was â€Å"saddened to find it in such a state† and ashamed that this was where he had come from. Hamid creates such an atmosphere to act as crucial for Changez’ realization of who he has become. Soon after Changez releases that his surroundings had not changed since his departure, he â€Å"had changed†. He realises that he, † was looking†¦ ith the eyes of a foreigner†¦. [of] that particular type of entitled and unsympathetic American†. This setting forces Changez to become aware of his new identity and question his loyalty to Pakistan. Changez feels as though he is â€Å"abandon[ing] his people†. Upon realising this, he begins to appreciate his home’s â€Å"enduring grandeur† and â€Å"rich†¦ history†. Furthermore, Hamid chooses Valparaiso as the next crucial setting because Changez associates the city with Pakistan, hence feeling more comfortable there.The city's decline as a great port reminds Changez of Lahore. Therefore he is able to trust the chief of the publishing company, Juan Bautisa. Thus, when Juan-Bautisa tells Changez of the janissaries', Changez is able to realise that he has become,† a servant of the American empire when it was invading a country of kinship to mine†. This setting and atmosphere provides the vital catalyst that convinces Changez that he no longer belongs in America. He now knows that his â€Å"days of focusing on the fundamentals were done†.Hamid uses particular settings and atmospheres in his novel, to provoke his protagonist Changez to feel certain emotions and act accordingly. The environments prove to be crucial in how the plot unfolds as they are all are essential in influencing Changez subsequent actions. The changes of New York City's welcomeness, his observations in Greece and his sense of comfort in Valparaiso all affect Changez emotionally. Overall, had the novel not used such specific surroundings, Changez feelings may have not been realised to their full intensity.

Dracula Essay

Good vs. Evil has long been expressed through movies and books throughout history. Batman beating the Joker, Spiderman banishing the Green Goblin, and Arthur and the guys defeating Dracula are all examples of literature based on the theme Good vs. Evil. In Bram Stokers, Dracula, Jonathan Harker represents the good, while the vampire, Dracula, represents the evil antagonist. One thing these four pieces share is that evil never fully overcomes good. They all start off as regular human beings, or on the good side, until their spirit and soul becomes taken over by a sense of evil. Gothic elements, such as, an atmosphere of mystery and suspense, supernatural events, gloom and horror, the tyrannical male, and the woman in distress are all present in these stories. The movie Interview with a Vampire, The Singing Springing Lark, and the story Interview with a Vampire can relate to Dracula because of their themes and gothic elements. The movie Interview with a Vampire is very similar to the story Dracula. It starts off in a dark night scene filled with dull and heavy music. Also, during the fight scene, the setting included thunderstorms and pouring rain. This shows metonymy because the dark and heavy music represents death and terror and the thunderstorms and rain were used to create a setting of darkness and fear. Other gothic elements shown in this movie were Lucy’s pale skin, the castle setting, and the exchanging of blood. In the Lindauer 2 story Dracula, Lucy’s skin starts to become very pale because she has been bitten by Dracula, which is what happened in the movie. This represents the gothic element tyrannical male, because of Dracula’s presentation of overpowering the other characters, more specifically Mina and Lucy. This directly relates to the gothic element women in distress because the women in both stories were treated poorly and were taken advantage of due to their weaknesses. In the story, The Singing Springing Lark, there are also many similarities to Dracula. For example, as in Interview with a Vampire also, most of the scenes take place in a castle. This shows the gothic element setting in a castle. As in most stories, a castle usually represents evil spirits and terror, which Dracula and the Beast both represent. Another gothic element shown is an atmosphere of mystery and suspense. In the story, the prince was hesitant to meet the girl’s family because â€Å"for if a ray from a burning light were to fall on him there, he would be transformed into a dove, and would have to fly with doves for seven years.† This is an important quote because in the story Dracula, the boys had figured out that Dracula’s evil spirit was weakest during the times of sunrise and sunset. If they didn’t get to him before the sun set, his powers were going to be restored. In other words, the change from dark to light represents a change from evil to good. In the story Interview with a Vampire, the plot line is almost exactly the same. For example, â€Å"The mere sight of a bared throat could bring about in me such a taste of arousal that I became like an animal, incapable of language or restraint.† This quote shows tyrannical male because it compares how Armand and Dracula are both similar in the way they kill their prey. They both attack where the blood is rich just like animals do and dominate their prey. It also Lindauer 3 shows a sexual side because the mere fact of killing got both of these characters aroused. For example, â€Å"With his left hand he held both Mrs. Harker’s hands, keeping them away with her arms at full tension; his right hand gripped her by the back of the neck, forcing her face down on his bosom. Her white nightdress was smeared with blood, and a thin stream trickled down the man’s bare breast which was shown by his torn-open dress.†(Stoker 283) This quote shows women in distress because it describes how Mina, in a sense, was raped. Just Armand, Dracula finds pleasure in killing, and transforming good into evil. One thing that all of these pieces share is that evil never overcomes good. Although evil makes its presence known and dominates at first, good prevails in the end. But since she had a small summary that went with what she was talking about, not a huge summary of the ientire book chapter by chapter, I understood just what she meant. In order to be able to advise P and Q it is necessary to examine the law in relation to short term licences. This will involve a discussion on the difference between a lease and a licence, and the different rights given to leaseholders over licence holders.
